Qualifications

• Must be a resident of Florida.

• Have computer or phone access for appointments. 

• Must be above 12 years of age (guardian consent required below age of 18).

• Paperwork and consent must be completed before your appointment. Consents are completed electronically! no physical paper to download or fill out; please contact our coordinator if you are experiencing difficulties. 

• Consultation is required on ALL patients.

Costs

• New patient visits $195

• Follow-up visits: simple visit $135, Complex visit or > 3months since last visit $150-175

• Medications vary, which are discussed during the consult.

Payment

• We DO NOT accept insurance.

• Payments can be made online or over the phone via a secure payment system. 

• HSA/FSA are accepted - Itemized and coded receipts available upon request!

• You may use your insurance for:

  • Blood work & Diagnostics​

  • Non-Compounded Medications

  • Reimbursement of out-of-pocket costs depending on your insurance contract (this varies between ALL plans)
