Paperwork and consent must be completed before your appointment. Consents are completed electronically! no physical paper to download or fill out; please contact our coordinator if you are experiencing difficulties. 

Consultation is required on ALL patients.

New patient visits are $450 includes initial hour visit and an hour follow up visit to review any testing and imaging ordered.

After your initial and follow-up visit we have two options going forward.

• Membership for Direct Care $75/month​

  • Follow-up $175 (45-60 minutes)

  • 20% OFF Fullscripts orders & nurse procedures

  • 10% OFF ALL retail products in-office

  • Includes monthly body analysis + nurse visit

  • Reduced rates on travel nurse visit $25 OFF 

  • Preferred pricing on aesthetics (Botox $10/unit)

• Non-Memberships; Fee for service structure. 

  • Follow-up $225 (45-60 minutes)

  • $25 Nurse visits

  • Travel nurse visits $100

  • Botox $12/unit

Medications vary, which are discussed during the consult.

We DO NOT accept insurance.

Payments can be made online or over the phone via a secure payment system. 

HSA/FSA are accepted - Itemized and coded receipts available upon request!

You may use your insurance for:

• Blood work & Diagnostics​

• Non-Compounded Medications

• Reimbursement of out-of-pocket costs depending on your insurance contract (this varies between ALL plans)
